| GERALD N. KADIS, M.D. |

Dr.
Kadis completed his undergraduate work at Hobart College in Geneva, New
York, graduating Cum Laude with highest honors in chemistry. He graduated
from the University of Rochester Medical School in 1971 and completed
his residency at the New York Neurological Institute of Columbia, Presbyterian
Medical Center in 1979 where he served as Chief Resident in Neurological
Surgery. Board certified by the American Board of Neurological Surgery,
he is also a Fellow of American College of Surgeons. A member of the Georgia
Neurosurgical Society, the American Association of Neurological Surgery,
the Tri-Area Medical Society Congress of Neurological Surgeons and the
Southern Neurosurgical Society, Dr. Kadis was awarded the Army Commendation
medal with honorable discharge in 1975 after serving as chief of professional
Services in U.S. Macthai, Thailand. Dr. Kadis moved to Thomasville and
founded the Institute in 1979 bringing over 16 years of clinical experience
to the practice.

| CRAIG A. FREDERICKS, M.D. |
Dr.
Fredericks joined the Institute in 1990 following his service as clinical
Instructor and Resident Supervisor in Neurosurgery at the University of
Southern California School of Medicine in Los Angeles where he was also
in private practice. he graduated from Stanford University with honors and
attended George Washington University Medical School. He completed his general
surgery internship at USC Medical Center where he was Assistant Resident
in Neurosurgery. Dr. Fredericks also completed residency training in neurosurgery
at Children's Hospital of Los Angeles and was Senior Resident in Neurosurgery
at Norris Cancer Hospital. At Los Angeles County/University of Southern
California Medical Center he completed residency training in neuro-radiology,
neuropathology, neurology and neurosurgery research. Dr. Fredericks is board
certified by the American Board of Neurological Surgery and is a member
of the Congress of Neurological Surgeons, the American Association of Neurological
Surgeons and the Georgia Neurosurgical Society. He has been published in
several neurosurgery textbooks due to his research in Stereotaxic surgery
and the optic system and brings over 8 years of clinical experience to the
Institute. |
| ROBERT H. LYNDE, M.D. |
|
Dr.
Lynde joined the Institute in 1993 following over 24 years of private
practice in Jacksonville and Tampa. He completed his undergraduate work
at Randolph-Macon College and graduated from the Medical College of Virginia.
He completed an internship at St. Joseph Mercy Hospital in Ann Arbor and
a residency in neurology at the Medical College of Virginia. He has served
as Clinical Instructor in Neurology at the Medical College of Virginia,
the University of Texas medical School at San Antonio and the university
of Florida Jacksonville Hospital's Educational program. He was also an
Assistant Professor of Neurology at the University of South Florida College
of Medicine in Tampa. Dr. Lynde is a member of the American Academy of
Neurology, the American Academy of Neuroimaging and American Medical Electroencephalography
Association. He is eligible for certification by the American Board of
Psychiatry and Neurology and has a special interest in neuro-radiology
and electrodiagnostic testing.
